New Technology in the Battle Against Transmission of Infectious Diseases Designed to Help Hospitals and Save Taxpayers Money

The new portable, self contained system requires no modification to the existing HVAC system and can be quickly and easily installed by on staff hospital personal. It comes complete with a negative or positive pressure HEPA filtration unit, digital differential pressure monitor and digital differential pressure monitor, and all the required hardware for set-up and operation.

The Safe Cell 800i borrowed much of its design criteria from its company’s predecessor the Safe Cell 100N, a rapidly deployable tactical military device that protects against airborne nuclear, chemical and biological accident or terrorism by creating a toxin free protected space in an ordinary room. The Safe Cell 100N is classified by the U.S. Government office of Defense Trade Controls as a tactical military device and Deemed a class 14 munitions.

The criteria was simple: the system must be in kit form and include all devices and hardware to create a selectable mode system for positive or negative pressure infectious isolation rooms, it must be easy to install by hospital staff in one hour or less, and must qualify the room to CDC guidelines for an infectious isolation room. Since most of the install time is used locating and controlling the existing air ventilation system for the Safe Cell, it’s possible that in the future, we will see hospitals being built without dedicated isolation rooms since every room can be converted so quickly.We may see hospitals with connections and vents in every room to immediately deploy the Safe Cell at the first indication of an airborne disease.

A standard unit will sell with the monitor for less than $4000, it costs an established hospital between $40,000 and $80,000 to remodel an existing room into isolation. Another great attribute of this machine is that it can be utilized with many of the surge capacity tents, trailers and facility conversions set up for mass casualty scenarios and fundable by a Homeland Security Grant.

On July 29, 2005, "key director" Ronald Matricaria, who had been chairman of Cyberonics board's Compensation Committee and a member of its Strategic Advisory Committee, resigned effective July 23. $600,000 annually base that can be adjusted;an annual bonus; a grant of 75,000 restricted shares vesting at a rate of 15,000 a year for five years; and on the first anniversary of the execution date of the employment agreement, a grant of 75,000 shares of restricted stock vesting at the end of each of four years at 18,750 per year;and finally, on the second anniversary, he will get another 75,000 shares of restricted stock vesting at a rate of 25,000 shares at the end of each of three years.

Under these circumstances, at the end of his third year of employment, Mr. Cummins will have the ability to trade 58,750 shares of restricted stock. At the opening today, such stock would be worth about $2,163,633.58 plus his yearly salary and bonus placing him over $3 million per that year alone. If this defendant is terminated "under certain circumstances," he will receive a sum twice his base salary, an annual bonus equal to 100 percent his base salary plus acceleration of his stock rights. Also, in recognition of his "contributions" to the Company, this defendant will receive 25,000 restricted shares on the date of agreement of the contract.

Fibromyalgia: Waste Basket Diagnosis or Just Garbage?

There is a sham going o­n in American medicine and no o­ne wants to talk about it. Fibromyalgia is the most common cause of generalized, musculoskeletal pain in women between ages of 20 and 55 years. There is no tissue diagnosis. No radiologic findings. No typical histology.

A patient walks into an office and complains of diffuse muscle pains of nonspecific nature in several different skeletal regions. No trauma was sustained. No bruising is present. The patient is not febrile and cannot distinguish whether the pain feels like it arises from the joint or the muscles themselves. Not unlikely, the patient also comments on their persistent fatigue and other nonspecific symptoms like abdominal cramping, on and off visual disturbances, dizziness, etc.

In his or her head, the doctor is thinking: somatizer. But in this day and age of malpractice and better-be-safe-than-sorry medicine, a full work-up is initiated -- sadly. Films are shot, labs are drawn, history and physical is repeated. Lo and behold, all is negative. Even the sed rate, (miraculously).

So what do we do as physicians? We reassure the patient that this is not a physical malady and suggest that this might be something stemming from their psyche, right? Wrong. We make up a diagnosis so we have something to label it and thus, something to treat. Now, suddenly there are mavens of this fictional disease. There are conferences dedicated to it; text books authored about it; medications designed for it; TV and radio commercials exploiting it.

Not me.

I won’t be a part of it any more. It's out of hand, and a recent study is helping delineate this point. Fibromyalgia is the most common cause of generalized, musculoskeletal pain in women between ages of 20 and 55 years; in the United States, the prevalence is approximately 2 percent and increases with age. The cardinal manifestation of fibromyalgia is diffuse musculoskeletal pain. Plus, a variety of poorly understood pain symptoms, including abdominal and chest wall pain and symptoms suggestive of irritable bowel syndrome, pelvic pain and bladder symptoms of frequency and urgency suggestive of the female urethral syndrome or of interstitial cystitis.

Fatigue is present in more than 90 percent of cases and is occasionally the chief complaint. There is no tissue diagnosis. No radiologic findings. No typical histology. How, then, can this be classified as a physical illness? I was taught way back in medical school that if it smells like a duck, and walks like a duck, it's probably a duck. Well, I'll tell you what this smells like…

A recent study, reported by the Associated Press, is shedding some light on the true nature of fibromyalgia. The study looked at "real" acupuncture verses improperly applied acupuncture (accu-placebo) in patients with fibromyalgia.

Now, it wasn't surprising to us that real acupuncture failed to show benefit versus fake acupuncture. And this fact did not lend credence to our theory of fibromyalgia as a fictional disease. Rather, it was that all patients, in both arms of the study showed dramatic improvement in just o­ne or two sessions, a much shorter treatment course than expected.

Whether the treatment used was real or fake made no difference in the study participants. Their symptoms improved greatly just after one or two sessions suggesting that just enrolling in the study may have been the best treatment course. Or, perhaps, the attention that was given to their "disease" improved symptoms.

Adam Burke, an acupuncturist who treats patients with fibromyalgia and was not connected with the study, said the findings were not conclusive, nor was the treatment the study's subjects received consistent with real-life alternative therapies for the condition.

Typically, such treatment can consist of more than a year of acupuncture, along with complementary therapies like herbs and lifestyle changes, he said. Also, acupuncture involving electrical stimulation that has been effective for fibromyalgia wasn't looked at in the study, said Burke, an assistant professor of health education at San Francisco State University.

Of course, they should be treated for more than a year. I'm sure Mr.Burke lets them know that right from the start of treatment.

Are we too cynical? Possibly. But you show me a patient with no physical exam findings, no radiologic abnormalities, no tissue diagnoses, and no gold standard of diagnosis, and I'll show you a patient that has an excellent physical prognosis.

While this practice has come under great scrutiny in recent years, it is still the rule rather than the exception, especially in competitive drug markets like cardiovascular disease and diabetes.

Drug reps are known in healthcare circles for their common practice of bringing lunches and free swag as a way of getting healthcare professionals to listen to their sales pitch. While hospitals are littered with pens, pads, and other gimmicks adorned with pharmaceutical logos, this sales practice seems outdated and juvenile for a field that has moved so far towards evidence-based medicine.

StopPagingMe.com, which bills itself as The Online Housestaff Community, has developed into a cultish portal on the web for young physicians across the country. Their often biting commentary on current medical news and original articles on everything from investing, to gifts for doctors, have won them literally thousands of fans.

Their most recent endeavor into sampling the opinions of the training physician community give an interesting perspective into the world of drug prescribing practices. Do these aggressive techniques of simply "getting the name out there," while the doctors are still young, have an effect on what a patient picks up from the pharmacy?

In a survey of 101 resident doctors about the drug companies' efforts to sway their opinion, 49 percent said that they see the drug reps at least once per week, and a full quarter of those surveyed reported that they saw reps on a daily basis.

While less then half found them "helpful" or "educational," significantly more found them "irritating," and nearly 15 percent found their mere presence "immoral."

Further questioning revealed that more than 25 percent of those participating in the survey believed that drug reps should not be allowed in the hospital at all. Fifty-four percent appreciated the free food offered by reps but preferred to do without the education that comes with it.

When asked about whether the doctors-in-training felt guilty about taking drug company swag, 64 percent said that they did not feel any guilt or find it unethical in any way.

The “…samples [are] useful for patients,” that cannot afford them otherwise, quoted one respondent.

On the other hand, 10 percent answered that they never take the drug company freebies and find the practice to be “unethical,” and “should be illegal.”

The most important question of the survey tested the drug companies’ efforts directly. They asked the residents if they ever feel the influence of the drug reps when writing prescriptions for patients. While 54 percent said that they “never” feel the impact of free lunches and colorful pamphlets, 35 percent answered that it occasionally may influence their decision of what medication to choose.

“It makes me think of the various options out there,” quoted a participant.

The drug reps “… only influence my decision if the drug would be beneficial to my patient. I would NOT prescribe a drug if I question its efficacy in a patient just because I was given a free lunch and a gadget.”

Those young doctors who remain opposed to the drug reps’ presence in the clinics, private practices and hospitals across the country, wrote vehemently about the weak-minded physician that would allow someone of much lesser training and medical background to compromise their medical management. Words like “never” and “useless” were scattered among clichés like “I do what’s best for the patient.”

But one, more practical, doctor mused, “I don't think it has an effect, but I know they wouldn't be there if the companies didn't know that the freebies and their presence works. They do studies to be sure their budget is well-spent.”
