Background
Stress testing the cardiovascular patient or those suspected of cardiovascular disease has remained a staple of diagnostic cardiology for decades. The predominant method of inducing physiological stress for the purpose of observing cardiovascular adaptations utilizes a treadmill and one of several empirically validated protocols. It has been established that many patients are unable to complete a standard treadmill stress test for a variety of reasons including deconditioning, comorbidities, orthopedic limitations, etc. It is hypothesized that stress testing using alternative methodologies may allow a larger percentage of the patient population to be tested and to reach a maximal (clinically relevant) test.
Methods
80 subjects with severe cardiovascular disease (post revascularization, CHF, or history of acute events) were enrolled in this clinical cohort study. Subjects at the same disease level who had previously completed a treadmill based exercise stress test at the South Carolina Heart Center (Columbia, SC) using the Bruce exercise protocol upon entry to cardiac rehabilitation were enrolled retrospectively to compose the control group for this analysis (n = 40). The experimental cohort (n =40) was comprised of subjects having clinically severe cardiovascular disease that were stress tested using the RHE cycle and the RHE exercise stress testing protocol.
Data analysis were conducted at the South Carolina Heart Center and descriptive statistics as well as two-sample T-testing for cohort comparisons of like variables were employed. Statistical significance was attained at p .05.
Findings
· The RHE cycle and stress testing protocol allowed a significantly higher percentage of subjects with clinical cardiovascular disease to achieve a maximal stress test when based upon RER (RER > 1.1).
· The mean percentage of maximal RER attained was significantly higher among the experimental cohort.
· The RHE cycle and stress testing protocol allowed a significantly higher percentage of subjects with clinical cardiovascular disease to achieve a maximal stress test when based upon HR (HR > 90% of age predicted maximum heart rate).
· The mean percentage of maximal HR attained was significantly higher among the experimental cohort.
· The RHE cycle and stress testing protocol allowed 85% of subjects with severe cardiovascular disease to attain a maximal stress test.
· The RHE cycle and stress testing protocol allowed 33% more severely diseased subjects to reach a maximal stress test than the treadmill when max was defined by RER.
· The RHE cycle and stress testing protocol allowed 33% more severely diseased subjects to reach a maximal stress test than the treadmill when max was defined by heart rate.
· The percentage of VO2 max attained and mean absolute VO2 were not statistically different among cohorts.
· Anaerobic threshold was met at an earlier percentage of VO2 max among experimental subjects suggesting that these subjects may be working harder at earlier stages of the testing protocol.
· Subjects tested via the RHE cycle and stress testing protocol were able to exercise for a significantly longer time than those tested via treadmill.
· The RHE cycle and stress testing protocol caused a significant reduction in the number of submaximal terminations due to fatigue among this severely diseased population.
· Subjects with severe cardiovascular disease who were tested using the RHE cycle and stress testing protocol reported the test/protocol as being significantly easier/less demanding than the treadmill and Bruce protocol.
Quality Control Suggestions
Comments presented for improvement of the RHE cycle by tested subjects and research staff included:
· Seat adjustment is difficult
· Add safety straps to foot pedals to reduce the potential of injury
· Program test protocols into computer to eliminate manual adjustment
· Reduce EKG artifact
Conclusions
This clinical trial investigating the efficacy of the RHE cycle and stress testing system among a severely diseased population (i.e. cardiac rehabilitation, post revascularization, post acute events, etc.) serves to establish validity and diagnostic value of the RHE protocol and stress testing system across the spectrum of patients having severe cardiovascular disease. The RHE system allows a greater percentage of patients to reach a maximal exercise stress test with diminished perceived exertion, thereby reducing the number of submaximal terminations and the resultant need for pharmacological stress tests.

HOAG Hospital Automates Patient Check In With Galvanon’s Self-Service Technology

Neuropathy Mystery Solved. New Study proves electronic ReBuilder System Effective for Neuropathy.
Recent Study of 480 patients indicates 95% of those studied responded to ReBuilder Neuropathy home therapy system.

ReBuilder Medical, Inc. today released an independant study of 480 patients over a 3 year period. Results showed that compared to a placebo, the Anodyne and ReVitamed Infrared systems, and Pfizer's and Eli Lilly's drug therapies, the double blind study demonstrated a 95% response rate to the ReBuilder home therapy. (The competitive Anodyne and Revitamed equipment fared no better than the placebo, while the prescription drugs offered only a 45% improvement and serious side effects.)
Neuropathy is a major complication of diabetes and 60% of those with diabetes will develop the tingling, numbness, and pain of neuropathy in their feet and hands. Currently the most common treatment is drug related, but the side effects of drugs are many times worse than the disease. Pfizer was recently fined by the US government for the use of Neuronton for neuropathy. Other electronic devices (infrared) that merely heat the feet have been tried unsuccessfully. The ReBuilder uses a tiny electrical signal that mimics the normal nerve signal to wake up dormant nerves that were temporarily restricted due to temporary bouts of oxygen deprivation. Ileostomy Surgery Update – “J-Pouch surgery for colitis or polyposis is devastating when the result is eating just one meal a day and making countless trips to the bathroom,” says the physician who heads the Ileostomy Surgery Information Center in Los Angeles. “Unfortunately, most ‘J-Pouchers’ don’t know they can escape this misery,” he says.

“Some J-Pouchers have no social life,” says Schiller. “They’re embarrassed about all the trips to the bathroom. They’re afraid about the possibility of leakage. Many avoid parties and restaurants altogether. It isn’t unusual for someone whose J-Pouch surgery has turned out poorly to bypass promising career opportunities, or even avoid going to college. They won’t take the chance of being humiliated by an accident. This is the secret many J-Pouchers live with and agonize over every day. It is a lonely, frustrating life.”

Hetero Drugs Gets Sub-License for Tamiflu
The oseltamivir sub-licensing deal with Hetero allows the inventor and original producer's intellectual property rights to be respected, Roche said.

The agreement with Hetero covers India and other developing nations and allows the Indian firm to produce a generic version of the flu treatment specifically for governments wishing to create emergency stores.
Swiss pharmaceutical group Roche said that An Indian company, Hetero Drugs, has been granted the right to produce the antiviral Tamiflu, which is considered the first line of defense against the H5N1 avian flu virus, for poor countries taking measures against bird flu and a possible influenza pandemic.
Roche commented on Dec. 12, it has been negotiating with 12 drugmakers in several countries on sub-licensing of Tamiflu.
Roche said Hetero would be able to start supplying the drug in the first half of 2006.
Tamiflu, which has been in short supply worldwide, is considered effective against the potential outbreak of human-to-human transmission of the H5N1 strain of virus, which has been causing bird flu epidemic among poultry in Asia and Europe.
Most avian flu viruses are not infectious to humans. Should an avian and a human influenza virus co-infect a human or pig, the virus strains can join mutate and create a completely new virus, which may be transmissible from animals to humans, and from humans to humans.
The oseltamivir sub-licensing deal with Hetero allows the inventor and original producer's intellectual property rights to be respected, Roche said.
"The recognition of intellectual property in India through this partnership with Hetero therefore is very significant as it respects the new Indian Patent legislation," Roche said.

Florida Physician Finds Possible Influenza Treatment From Elderberries
Dr. Glenn Knox, a Clinical Professor at the University of Florida, theorizes that elderberries are effective against the flu due to a specific substance present in the elderberry plant.

Knox notes, "further research on the properties of SNA-I is needed to determine how useful it is in the treatment of influenza. The possibility that SNA-I may offer a therapy for influenza is very exciting."
